Under no circumstances are players or parents to use the team email addresses for personal use. Team use must be approved prior to sending an email and must be approved by a coach with authorization from the organization director. Any unauthorized use of team email addresses by parent/player is a violation of team policy and is cause for removal of the player from the team without refund. Use of unauthorized email is illegal and any damages to the team caused by unauthorized use of team email addresses is the responsibility of the individual involved.
Authorization to Use of Photographs and Images.
I do hereby grant permission to the Clubs. And its owner(s), officer(s), trustee(s), employee(s), agent(s), representative(s), successor(s), licensee(s) and assign(s) to photograph the image of the registered player herein, likeness or depiction. I hereby grant permission to the clubs to edit, crop, or retouch such photographs, and waive my rights to inspect the final photographs. I hereby consent to and permit photographs of the registered player herein, father and mother, siblings, and or those of my minor children to be used by the Clubs worldwide for any purpose, including educational and advertisement purposes, and in any medium, including print and electronic. I understand that the clubs may use such photographs with our without associating any names thereto. I further waive any claim for compensation of any kind for the clubs use or publication of photographs of me, the registered player or those of my minor children. I hereby fully and forever discharge and release the club from any claim for damages of any kind (including, but not limited to, invasion of privacy; defamation; false light or misappropriation of name, likeness, or image arising out of the use or publication of photographs of me, the registered player herein or those of my minor children by the clubs, and covenant and covenant and agree not to sue or otherwise initiate legal proceedings against the clubs for such use or publication on my own behalf or behalf of the registered players herein or on behalf of my minor children. All grants of permission and consent, and all covenants, agreements and understandings contained herein are irrevocable.
Refund Policy:
HotStix Lacrosse Club Team Refund Policy:
It is the Clubs policy that registration and team fees are final and non-refundable. Hotstix will not refund any portion of your tryout or registration fee for any reason. Hotstix registers each player to fill a specific spot on the clubs roster and since these spots are limited, it is up to the player/parent to make the commitment to participate. In addition, the information that is posted regarding which tournaments/practices/clinics we will participate in and the assciated dates are not finalized at the time of registration. Therefore changes or elimination of any tournament or practice/practices/clinics dates may be necessary and will not be a cause for a credit or refund.
Hotbed Lacrosse Camps Refund Policy:
If, for any reason, you are unable to attend a camp for which you have registered, a credit will be issued for all moneys collected. This credit can be used by you or any family member for any HotBed Lacrosse Camp/Clinic/HotStix Team through the end of the following year the credit was issued. For example, a credit issued in May, 2011 is valid through the end of 2012.
If the camp is cancelled or amended in any way due to inclement weather conditions such as rain lightning or excessive heat, no refund will be provided for the part of the camp that is missed. We will attempt to schedule additional time in the same week if possible.
In the event a participant must leave the camp early and cannot return, a pro-rated credit will be issued that can be applied to the fee/fees for any Hotbed Lacrosse Camp/Clinic/HotStix team through the end of following year.

I authorize the clubs to utilize my email address and my phone numbers for communication purposes. These communication purposes include informational as well as promotional purposes for the clubs.
I acknowledge and represent that I am over the age of 18, have read this entire document, that I understand its terms and provisions, and that I have given up substantial rights by signing it and sign it voluntarily. By registering my child for a lacrosse camp, clinic or team I am giving my consent and electronic signature. I understand that agreement with all of the above terms is necessary to play on a team.
